New patients, coming in for their first visit, are required to arrive earlier than their scheduled appointment time due to the time it takes to:

1. Fill out the new patient paperwork
2. Allow our staff to scan the paperwork and enter demographic information

Arrive thirty (30) minutes early if you have NOT downloaded, and pre-filled out, the paperwork from the web site.

Arrive fifteen (15) minutes early if you have already downloaded, and pre-filled out, the paperwork from the web site.

New Patient forms are available for download once you have been approved as a patient and scheduled your first appointment. Filling out the New Patient Paperwork saves time on your first visit. Otherwise, please come 20 minutes early to your first appointment.

Patient Logs for tracking Blood Pressure, Blood Sugar, Exercise and Diet are available for download. Patients, please fill in the columns that apply to you and return to Dr. Carney's office by bringing to your next appointment, sending via regular mail or faxing to (512) 295-4777.

There are two versions of the form. One is a PDF file you can print and write on. The other is an Excel spreadsheet you can fill in with your computer and then print.
